The answer is... The Reflecting Skin, starring Viggo Mortensen, around 1990 I think.
Small cast, vast open grain fields as a backdrop, a sociopathic child abused by mother, father with pedophilic past, brother comes home from war and befriends/falls in love with lonely widow next door and oh yeah, there's a carload of thugs looking for a kill... Things go awry as the young boy sinks deeper and deeper into psychosis...
